Your Edge Cloud forms provide you with detailed information you need to pass on to your patients.

Online Forms provide a way for patients to fill out forms electronically, and a way for you to use that data in patient folders, merge letters and subgroups. Your online forms can be of any type (health history, office questionnaire, outpatient interview, etc.). Your system initially includes a new patient health history form. You can request additional customized forms for a fee. In addition, you can include your own logo at the top of all online forms.

Your Edge Cloud labels provide you with an easy way to print mailing labels, chart labels, and more for your patients, responsible parties, professionals, insurance companies, and employers.
