Patient Forms

Your Edge Cloud forms provide you with detailed information you need to pass on to your patients.

Click to OpenChoose Default Formats

Use the Default Forms section of your Reports and Forms editor to select the default format to use for paper forms. This is the format that will be selected when you run the form from the Forms option of the Home or Patient ribbon bar. If you have purchased customized forms, you may choose your form as the default. If needed, you can set up different default forms for individual offices and specialties in your practice. See "Reports and Forms Editor" for details.

Click to OpenAssign a Correspondence History Category to Forms

Use the History Category editor to set up the categories for sorting and filtering the forms and documents recorded in the correspondence histories and notepads of your patients, responsible parties, professionals, employees, and your practice. See "History Categories Editor" for details.

Click to OpenPrint Patient Forms

You can print all patient forms from the Forms section of the Reporting window. In addition, you can run the Quick Reports version of several forms from the Forms menu on the Home and Patient ribbon bars.
